Report conditions, not just the stain
Note the first observed time, rain conditions, ceiling location and any recent HVAC, plumbing or rooftop work. Record whether the problem persists after the weather changes. Photographs from safe interior locations are useful; do not enter restricted roof areas to collect them.

- Which roof and equipment details were inspected.
- The evidence for the suspected entry route.
- The proposed compatible repair and any temporary measures.
- How unresolved moisture or recurring leakage will be followed up.
